DESCRIPTION: \nAdd a 2027 date to your diary!\nThe International Conference on Integrated Care (ICIC27) in partnership with&nbsp;<a href="https://ijic.org/" target="_blank" rel="noopener noreferrer" data-auth="NotApplicable" data-linkindex="0">The International Journal of Integrated Care (IJIC)</a>&nbsp;and the&nbsp;<a href="https://www.sm.ee/en" target="_blank" rel="noopener noreferrer" data-auth="NotApplicable" data-linkindex="1">Ministry of Social Affairs of Estonia</a>&nbsp;will take place in Tallinn on 10-12 May 2027. The conference will bring together leaders, researchers, clinicians, managers, community representatives, patients and caregivers from around the world who are engaged in the design and delivery of integrated health and care.\nICIC27 will explore how integrated care can move from ambition and structural reform towards measurable impact for people, communities and systems. As health and care services face growing demand, workforce pressure, financial constraint and widening inequalities, the need to demonstrate the value of integrated approaches has never been greater.\nThe main conference theme is&nbsp;From Integration to Impact -&nbsp;Delivering better outcomes through data, coordination and population health. It&nbsp;invites participants to examine how better data, stronger coordination, digital innovation and population health approaches can support more proactive, equitable and sustainable care. ICIC27 will focus on how integrated care can improve outcomes, strengthen resilience and create better value across health, social care, public health, and community systems.Four conference themes include:\n\n\nFrom Data Silos to Shared Intelligence: Digital Information as an Enabler of Integrated Care\n\n\nMeasuring what matters: Outcomes, Equity and Population Health in Integrated Care Systems\n\n\nSystem Resilience through Integration: Preparing for, responding to and recovering from Crises\n\n\nMaking the Case for Care Coordination: Demonstrating value and system efficiency in integrated care\n\n\nMore information about the themes is available on the <a href="https://integratedcarefoundation.org/conference-pages/icic27-conference-themes" target="_blank" rel="noopener">conference website</a>.\n\nWatch out for the ICIC27 call for papers!
