Why the project

i2X is deeply committed to improving experiences for both health professionals and patients, by optimising workflows and integrating cutting-edge technologies. It supports industry preparedness, offering a safe and structured space for X-Format adoption in clinical settings—primarily in public or publicly-oriented institutions. By bringing together numerous digital health companies, i2X fosters innovation and inclusiveness. It amplifies the voices of all key stakeholders, especially members of the overstretched health workforce.

As an ambitious, highly impactful initiative, i2X will tackle significant challenges, including interoperability gaps and resistance to digital transition.

The project will focus on demonstrating real-world benefits for patients and citizens, improving system efficiency, reducing costs, and providing actionable recommendations for successful European Electronic Health Record Exchange Format (EEHRxF) implementation.

Project description

i2X stands for “A Common Electronic Health Record Format for Better Health Across Europe”. i2X is a flagship initiative that is involved in three key activities: it contributes to the goals of the Digital Europe Programme, aligns with the Single Digital Market strategy, and supports the ambitions of the Digital Decade 2030.

At the heart of i2X lies the EEHRxF, the X-Format, a key component of the European Health Data Space Regulation which entered into force on 26 March 2025.

The X-Format is crucial for both the primary use of health data (supporting individual care) and secondary use (e.g., policy-making, research, and innovation). i2X will showcase how an “intelligent” implementation of the X-Format—supported by tailored IT solutions and positive user experiences among health professionals—can drive real change in European healthcare systems.

i2x launch

The project’s goal is to demonstrate sustainable, real-world adoption of the X-Format across several data priority categories. More than 35 live demonstrators will run in 14 healthcare settings across 10 EU Member States. Thirty-eight partners are involved, and a growing number of others may join soon.

These in-service live demonstrators will prove that the EEHRxF can enhance health and care pathways without placing additional burdens on health professionals or systems. The project will generate practical insights and best practices that help accelerate adoption of the X-Format across Europe and promote interoperability among healthcare organisations and systems.

i2X is a hands-on implementation project, experimenting in real clinical environments to explore improved methods for capturing, exchanging, and using health data. i2X will deploy advanced digital tools that ease the everyday work of health professionals. As a result, the project will:

  • Deliver evidence, policy recommendations, and technical guidance.
  • Promote alignment with EU standards.
  • Create opportunities for other organisations to join as early adopters.

The project plays a key role in advancing open innovation policies and in implementing MyHealth@EU services. Building on the foundations laid by other EU initiatives—such as Xt-EHR, XpanDH, and xShare — i2X will seek synergies with the  myHealth@myHands project.

EHTEL’s role

EHTEL is glad to be involved in i2X as an associate partner. EHTEL is proud to have two EHTEL members who contribute directly to the project: Tic Salut Social and CHU Liège. EHTEL has offered to add to i2X’s communications and dissemination by spreading the project’s news and developments. We add to i2X’s network and ecosystem by bringing on board our own members and their own networks.

Teamwork

Over the next four years, 12 EU Member States will participate in i2X. They are Belgium, Cyprus, Czech Republic, Finland, France, Germany, Greece, Ireland, Italy, Portugal, the Netherlands, and Spain. Most will develop demonstrators, some will integrate AI-based technologies. At least 1,200 healthcare professionals are expected to be engaged in the demonstrator sites. Ultimately, more than three million European citizens/patients will benefit from the project’s activities. We anticipate that EHTEL members—besides TICSalutSocial and CHI Liège—will have the opportunity to contribute to the project, especially through showing your roles as early adopters of electronic health records and digital health solutions. Your involvement will assist in the goal of achieving:

  • An up to 80% improvement in the availability of high-quality, structured health data in electronic health records—both nationally and cross-border.
  • 75% increase in patients’ access to their health data.

Perspectives and next steps

i2X is at its momentum-building phase. Its website www.i2Xhealth.eu is already online—it will act as the project’s central hub. Through its LinkedIn group, the project’s work has been very proactive in spring/summer 2025.

Representatives from I2X have contributed to a number of major events, including HIMSS Europe (Paris, France)  and the IHE Plugathon (Vienna, Austria).

Several national clusters are being formed under i2X—Greece will act as the first pilot initiative.

While i2X will continue to publish news in the English language, it also plans to showcase and promote plenty of other multi-lingual information. So, look out for info in your own country’s language or languages!
